WELCOME TO DWIGHT MORROW HIGH SCHOOL AND THE ACADEMIES @ ENGLEWOOD

All new students must submit the following documents prior to starting classes

1. PROOF OF IMMUNIZATION

If you are transferring from another district in New Jersey, please submit the health record (A-45) from the prior school's Nurse. If you are unable to obtain the A-45, please send a copy of your immunization from your pediatrician or clinic.

If you are transferring from out of state or out of country, an official immunization record from your prior school or physician’s office is preferred. If you are unable to do this, the immunization record that your parent has will suffice.

The School Nurse will review all immunizations before students are given a schedule. If a student requires additional immunizations, he/she will be given a 30 day grace period to obtain those immunizations. If those immunizations are not given, the student will be excluded from school.

The student’s parent/guardian is responsible for completing this form at the time of new student registration and whenever any medical changes occur. This form will be reviewed by the School Nurse and all student health alerts will be identified and if necessary, clarified with the parent/guardian and/or licensed health care provider.

Please also contact this office if there is a chronic medical condition of which the Nurse should aware. Accommodations can be made to help your child in school as needed. Also make sure to update the Nurse of any new allergies (food or medicine) or medical conditions affecting your child. It is important for the Nurse to maintain accurate medical records.

3. RECENT PHYSICAL EXAMINATION

All physicals must be from within the last year (365 days). There are two forms that may be completed by your child’s medical home (pediatrician or clinic). Please include all immunizations that your child has received.

Sports Physical Forms 

  • All four pages must be completed. The student and parent/guardian must sign the bottom of pages 1 and 2. The physician, physician’s assistant (PA), or nurse practitioner (NP) must complete and sign pages 3 and 4. There must be an official office stamp on page 4.
